DAYTON, OHIO — Air Force officials at a southwest Ohio base are notifying thousands of pharmacy customers about a possible mix-up in prescription refills because an automated system may have added the wrong pills to at least one medication.
A spokesman for Wright-Patterson Air Force Base near Dayton says the problem apparently occurred at the base’s Kittyhawk Satellite Pharmacy. It refills about 1,000 prescriptions a day for active and retired military members and dependents.
Spokesman Daryl Mayer says anyone who got refills there between April 23 and April 29 should call or have them checked at the pharmacy or Wright-Patterson Medical Center. He says one person had reported getting wrong pills as of Wednesday, and officials believe only a small number may have been affected.
Mayer says no adverse reactions have been reported.
